All Hopes Incorporated is a Housing with services and Assisted Living Healthcare provider, providing group home accommodation and services in residential setting. All Hopes Incorporated is licensed by Minnesota Department of Health and Department of Human Services to provide housing and programs for clients on CADI, TBI and waiver services. We serve people with Severe and Persistent Mental illness, Intellectual/Developmental Disabilities, Autism Spectrum Disorders, Behavioral and emotional challenges, Brain Injuries, Physical and medical Disabilities. Housing with services and assisted Living settings are provided in single family homes with maximum of 4 residents. Clients have private rooms.

OUR PRACTICE
The founder of All Hopes Incorporated has over 27 years’ experience working with patients’ various disabilities including Mental Health, medical and physical disabilities in hospitals and long-term care and have spent the last 10 years managing Mental healthcare and transitioning patients to the community. While in graduate program, coupled with the experience of working with patients in mental Health illnesses, the founder developed a Model to improve care of patients with mental health illnesses and disabilities. Welcome Model is used as our practice model in providing care for all clients using our services.
Welcome Model – Welcome Model is a practice guide that introduces trauma informed care and aggression prevention pyramid, underpinned by Margaret Neuman’s Theory of Health as Expanding Consciousness as a theoretical foundation to provide a new paradigm in Mental health care with a broad view and understanding of client’s safety and relationship building. Welcome Model prepares our staff in understanding the client’s pattern and the collaboration with clients and families in recognizing each person’s transformation into their current pattern, to find greater meaning in life and how to relate and interact with the present, the environment and the future.

Housing are provided in single family homes with 4 Bedrooms and private rooms for each client.
